We are very excited to announce that beginning Easter Sunday, April 04, we will begin taking requests for our new Name Badges. Simply fill out the form below, hit submit and the Administration Office will prepare a new name badge for you. Since this is a new process, please allow us 2-3 weeks
to complete. After that time, you can pick up your Name Badge at the Welcome Center. If you would like to store your Name Badge at church,
space is available in the hallway behind the Welcome Center.
If you have further questions, please contact the office at info@mccgsl.org.
Your phone number and email will not appear on your name badge
but is required information should the office need to
contact you about your form.
