Simply fill out the form below, hit submit and the Administration Office will prepare a new name badge for you. Since this is a new process, please allow us 2-3 weeksto complete. After that time, you can pick up your Name Badge at the Welcome Center. If you would like to store your Name Badge at church,
space is available in the hallway behind the Welcome Center.
